腸液糞便積腹腫 8旬翁罹疝氣險喪命
(健康醫療網／記者林怡亭報導)
一名85歲老翁，因中風後失語，去年被家人發現右腹靠近髖骨處，出現直徑3公分的一塊發黑皮膚，緊急就醫檢查，醫師起初懷疑是蜂窩性組織炎導致，但以刀片劃開皮膚清創後，腸液與糞便竟從傷口流出，隨即安排電腦斷層檢查，發現患者罹患罕見的嵌頓型閉孔疝氣，已有致命危險，緊急開刀治療，所幸術後患者恢復良好。
皮膚發黑流出糞便 罕見型疝氣險喪命
這名老翁的腸道，因被閉孔疝氣卡得太緊，促使部份腸壁壞死，甚至形成腸道皮膚瘻管；臺中慈濟醫院一般外科醫師高國堯指出，透過大範圍清創，並手術切除壞死腸段，再修補閉孔疝氣，由整形外科接手，以兩階段植皮，住院一個月後，患者順利出院。
閉孔疝氣好發族群 以年老瘦弱居多
根據文獻指出，閉孔疝氣好發族群，包括年老瘦弱、生過小孩的婦人等，主要因脂肪組織變少，使得閉孔神經及血管外的空隙變大，男女比例又為1比6；高國堯醫師表示，但此個案出現的嵌頓型的閉孔疝氣較為罕見，研判恐是因食欲不振，體重僅35公斤為主要原因，所幸患者壞死腸段感染的腸液沒有往腹腔流，即使腸道壞死，也未引起腹膜炎，是不幸中大幸。
嵌頓型疝氣導致腸阻塞 得緊急手術治療
閉孔疝氣發生率是所有疝氣的0.05到1.4%，屬於罕見疾病，加上沒有特殊病症，較難發覺，臨床上，僅兩成患者由觸診發現；高國堯醫師呼籲，通常除閉孔神經及血管外的空隙，都由脂肪組織填塞，小腸掉進去機率不大，但一旦不幸卡住，就會造成嵌頓型疝氣，導致腸阻塞，這時就得緊急手術治療，否則恐有生命危險，不得輕忽嚴重性。
